Unique and Anonymous Identifier

To track responses anonymously throughout the study, we will ask you at each stage to enter an eight-digit code that is unique to you. Your unique identifier will be composed of the last four digits of your phone number combined with your birth year. For example, someone whose phone number is (123) 456-7890 and who was born in 1990 will enter “78901990” as their unique identifier. Please make a note of your unique identifier now and take care to correctly enter it as you will be asked on numerous occasions to input this number. It is vital that you type the same eight digits each time you are prompted.
